REGISTRATION

Basketball Player Fee (Ages 4-17) - $150

Fees can be paid in person or through the link provided after registration.

• $40 deposit is due at registration

• $150 (minus deposit) due in full by November 1st

• Players will not be placed on a team until their balance is paid in full

NOTE: The uniform size requested on this form will be the size ordered for the player. If the size ordered does not fit, it can be replaced at the players expense. Size requests cannot be guaranteed for registrations after November 13.
